Insurance claim data suggests that musculoskeletal disorders dominate workplace health

According to an analysis of the private medical insurance (PMI) records of over 45,000 UK employees carried out by Aon Employee Benefits with its largest clients, the highest claims are for musculoskeletal disorders- almost double those for cancer related illness. In a study of reports from private medical insurers (PMI), Aon found that 31 percent of claims were for musculoskeletal concerns, while 15 percent were cancer related, 4 percent were for mental disorders and 4 percent for urology. The data forms part of its new report, Wellbeing: Examining the correlation between employee health and financial wellbeing. Among the remaining 46 percent of claims, problems included gastrointestinal issues, diagnostic and treatment planning (equally prevalent), followed by obstetrics, heart, respiratory, head/neck, trauma/injury, nervous system, and eyes, ears and dermatology.

A new study published in the British Medical Journal suggests that cycling to work could halve the risk of cancer and heart disease and that any active form of travel to work offers similar health benefits. Although the report does not claim to establish the direct causal links, the five year study of 250,000 commuters in the UK concludes that “cycle commuting was associated with a lower risk of CVD, cancer, and all cause mortality. Walking commuting was associated with a lower risk of CVD independent of major measured confounding factors. Initiatives to encourage and support active commuting could reduce risk of death and the burden of important chronic conditions.” The five-year study compared people who had an active commute with those who were mostly sedentary in cars and public transport. The report found that regular cycling to work cut the risk of death from any cause by 41 percent, the incidence of cancer by 45 percent and heart disease by 46 percent.

Toss a sliver of information into the great stream of accepted public narrative and see what happens to it. There it goes, briefly visible on the surface then consumed; part of the stream but no longer to be seen. A perfect example of this is provided by a recent piece of research carried out by the Texas A&M Health Science Center School of Public Health into the effects of standing at work on a small sample of call centre workers. While the results of the study are impressive, notably a 46 percent increase in productivity, by the time the story was reported on Inc.com, the 167 call centre workers had suddenly morphed into ‘everybody’. It should go without saying that the headline ‘Your Productivity Will Increase by 46 percent if You Stand at Your Desk’ does not reflect the conclusions of the original research. The statements by the researchers suggesting that the study is significant with regard to call centre staff but merely indicative of a wider issue go ignored.

employee-fitness-muffin-topsAs the national obesity crisis worsens and a regular flow of statistics inform us that we’re a nation in trouble, with no demographic escaping the threat, we seem to have become obsessed with how fit or unfit we are. This is reflected in our growing interest in how many calories we consume and how much exercise we manage – right down to how many steps we take a day. No surprise then that this interest has started to manifest itself in the workplace where employee fitness appears to be an issue addressed in the boardroom. Last year, research carried out by The Workforce Institute at Kronos revealed that almost 75 per cent of workers (a total of 9,000 were questioned) believe that wearable technology, designed to capture vast amounts of biometric data and manage health risks, could lead to increased efficiency and productivity in the workplace. Measuring levels of physical activity is but a small part of the obsession with health and well-being which has infiltrated the workplace overall. There is also physical evidence, from the desks that we sit at to issues of bicycle storage and showering facilities, that we are mindful of our health.

Work is greatest cause of stressSeventy-five percent of US employers say workplace stress is their number one health concern, but a disconnect between employers and employees on the causes of stress could undermine efforts to address the problem. Data from Willis Towers Watson’s 2015/2016 Global Staying@Work Survey along with its 2015/2016 Global Benefits Attitudes Survey reveals that the top causes of stress has picked by employer tended to fall into categories of large organisational issues, such as change and ubiquitous technology connections which can make employees feel that they are always on the job. By contrast, employees pointed more directly to specific elements of their personal work experience. For example, employees ranked organisation culture — including a lack of teamwork and a tendency to avoid accountability — third on their stress list, while employers ranked it last. Conversely, employers identified insufficient work/life balance as the top stressor for workers, while employees ranked it sixth.

Lunchtime habitsMany UK workers are under so much pressure they can’t escape their workplace for lunch, with almost three in five employees regularly eating at their desk. This could be having a detrimental long-term effect on the health of millions of workers across the UK. The results of a survey, commissioned by the National Charity Partnership, a collaboration between Diabetes UK, the British Heart Foundation (BHF) and Tesco, found that office workers say heavy workloads (32 per cent), workplace culture (14 per cent) and stress (13 per cent) are to blame for not getting away from their desks for a walk at lunchtime. Despite more than nine in ten employees reporting that being outside makes them feel healthier or more positive, more than half of workers who were questioned (52 per cent) never leave their office for lunch. Almost one in four (24 per cent) say they regularly work through their break.

WellbeingAlthough the vast majority of employers (89 percent) say that the wellbeing of employees is important to them, participation in corporate mandated health programmes remains ‘low’ with only 38 percent of employees worldwide participating in any wellbeing activity or health-related management programmes in the last year, according to research from insurers Willis Towers Watson. In addition, 41 percent of European employers say they lack the budget and resources to deliver an effective programme. The research claims that the top five health related issues of most concern in Europe are stress (74 percent), sedentary lifestyles (45 percent), presenteeism (33 percent), obesity (32 percent) and nutrition (31 percent). In response to these issues, organisations have initiated a range of programmes, the most common of which are onsite vaccinations (62 percent), diet and exercise (61 percent), health risk assessments (58 percent) and biometric screening (53 percent).

Kinnarps-sit-standWhile the UK, US, Australia and other nations continue to treat them as something of a novelty, across Sweden, Norway and Finland, over 80 per cent of office workers use sit-stand desks. Offering employees a height adjustable work station is now mandatory in Denmark. However, sit-stand working is still in its infancy in the UK, with only 2 per cent of similar workers having access to variable-height workstations.  Given the huge amount of news coverage devoted to the subject of sedentary lifestyles in the last couple of years, ‘sit-stand’ and ‘active working’ have become buzz terms in UK workplace design. The ‘On Your Feet Britain’* campaign has raised awareness of the health perils risked by the many Brits who spend an average of 8.5 hours a day sitting, whether at their desk or slumped in front of the telly.  Inevitably, savvy employers will be asking themselves if they can afford to ignore the problem.

A study published this month in the journal Occupational Medicine suggests that buildings designed to promote active workstyles have a positive effect on the health of occupants. The research, led by Dr Lina Engelen of the University of Sydney, set out to explore whether an ‘active design’ office increased the physical activity, productivity and mindset of occupants. Although a small scale study with just 34 employees working in four locations at the University, the results suggested that people responded to the active design of the spaces by spending less time sitting and more standing and consequently reported lower levels of back pain. However, there was no improvement in productivity or physical activity. The research was based on 60 percent of people working in open plan areas, compared to just 16 percent before. Other studies have shown that sedentary work is linked to a wide range of ailments including heart disease.

Outstanding Landscape of Affordances 3As we’ve always argued, the now commonly parroted idea that ‘sitting is the new smoking’ is extremely dubious and has led to a degree of hysteria about the effects of sedentary work and the substitution of one harmful extreme for another. Now a new peer reviewed study from researchers in the UK and Australia confirms earlier findings that suggest that what is important is not posture or position, but movement. The study of 5,000 civil servants over a 16 year period was carried out by researchers from the University of Exeter, University College London, and the University of Sydney (Australia) and funded by organisations, such as the British Heart Foundation, Stroke Association, the National Heart and Lung Institute, and the National Institute on Aging. It concludes that ‘no associations were observed between any of the five sitting indicators and mortality risk’.

workplace design and how we workIn years gone by, a ‘one size fits all’ approach to office design might have been the norm, but as the decades have progressed, so too have the options available to businesses designing ‘homes from home’ for their office-based workforces. As new interpretations of the office environment proliferated, so the open plan model came to into being and eventually evolved into the default office design model. This initially brought greater variety than ever before but, ultimately, a one size fits all mentality in workplace design ultimately prevailed – every worker was expected to work in certain ways, utilising the equipment and furniture supplied and designed for them. From inception through to occupancy, the average new office involves a six-year period of design and construction involving varying teams of people discussing the best and most attractive solution for the actual end user.
